After you pass the first three islands, how do you get to the rest of the islands? Is it possible in America?

You need to have beaten the Elite 4 and have at least 60 Pokemon caught. Talk to Prof Oak and he'll give you the National Pokedex. You'll need this because on the rest of the Sevii Islands are wild Johto Pokemon for you to catch.

Only up to Seven Island though. To get to Navel Rock and Birth Island you'll need Mystic Ticket and Aurora Ticket respectively from a nintendo wonder spot (such as NYPC)

After you ctach 60 pokemon and talk to professer Oak how do you get to Four Island?

You have to get the Ruby Plate and give it to the man in the Pokemon Center on Island 1 who is standing by the machine. He will give you something like a Rainbow Pass which allows you access to Islands 4 - 7.
You get the Ruby Plate on Island 1. In the mountain (where you can go to Moltres) where the Move Tutor who teaches Explosion is standing are two Team Rocket members. Fight them and you can enter a cave which leads to the Ruby Plate.
