The Japanese government admitted that they did not properly prepare for such a situation...very rare for them to admit something like that.

No, it is part of normal procedure to have several plans for such a situation. I live 9 miles from a nuclear plant which I know has at least three different plans for handling containment failure.
The Japanese government was disorganized in regards to disaster recovery and evacuation procedures. A country that exists along the Pacific volcanic ring is well aware of the impending dangers that earthquakes pose. Much like New York City during 9/11, there was a lack of communication and confusion.
